This page is a place to keep track of resources you may want moving forward. For your own reference, keep this page updated with resources you think you will want to keep track of. Still give a bit of context here, for others who may find this page as well.

Evidence & Research

Replace this text with the title and source of the resource. In two or three sentences, say what it is, where it came from, and why you want to be able to find it again.

Then say how you would use it. When would you reach for this in clinic, and what does it help you do? A few sentences is plenty.

Patient Education Materials

Replace this text with the name of the handout, video, or visual and where it came from. Note who it is for and what reading level it assumes. 

 

Add anything you would change before giving it to a patient, and note which languages it is available in.

Assessment Tools

Replace this text with the name of the form or tool and where it came from, such as a diet history or a 24-hour recall.

Say how it fits into an appointment: how long it takes, what you would ask alongside it, and what you would do with the answers.

Professional Organizations

Replace this text with the name of the organization, journal, or continuing-education source and what they publish. 

 

Add the link and a line on why you will come back to it: new guidelines, CE credit, or practice resources.
